expedite your first visit, we encourage you to:
A. Schedule your appointment
as soon as possible, call (817) 571-1666
B. At the time you schedule your appointment, request the delivery of our
Dental Registration and History forms, or you can email us directly at -
cyberdalton@earthlink.net
and we can send it to you via email.
C. Either bring the paperwork with you to your
appointment, or fax it to our secure fax site at
(817) 571-1669.
D. The sooner we get you
information, we can begin to customize treatment especially for you,
and lessen your wait time in the office.
